(pain tolerance can differ from human being to soul, different cultures etc. ) Provide soothing enviorment, adminsiter analgesics, eval response, possible side effects. Instruct of self soothing methods, relaxation technique, misery managment.
Altered Urinary Elimination: related to physiological return to non-pregnant state. Decreased circulating blood flow, increased renal plasma flow.
Note frequency, urgency, incontinence, nocturia, force of stream, spasm, bladder spasms, flank niggle, etc.
observed for ss infection-cloudy, foul odor, bloody urine. Check for bladder distention, retention. Instruct on regular voiding rota.
Constipation: related to poor muscle tone, analgesia use. etc.
Assess color, consistency, amount and frequency of stool. auscultate abdfor presence, location and characteristics of bowel sounds. assess medication use, analgesia, laxative, emena use. palpate abd for distention. Instruct on dignified fiber diet, increase in fluid intake, Instruct on ss impaction, Encourage mobility and instruct on impact of immobility.
